I looked at the Wake county mental health release form for the CHP application....from all the legalese, what I got is that they want your doctor(s) names and contact.

I don't have a family physician that I see....when I get sick, I go to a doc in the box (or just tough it out if it's a cold/sinus infection) and the only doctors I've seen in the past few years are an Ear, Nose & Throat doctor, a sports medicine specialist for joint issues and a radiologist for an MRI about the joints.

Will I need to just use those specialists? Or put nothing? Will I be denied if I don't have a family doctor? If so, I guess I need to get a physical?

Put nothing.....ir no family doctor. They're gonna check you against all the local medical facilities anyways.

Just leave it blank. I don't have a doctor either. Wasn't an issue. I don't know where they check for mental health; I suppose some state records or something...

Nope, leave it blank or put none. I had the same question when I applied and asked while I was filling out the application. They said it was no bid deal.
