Edited to add: First step is actually to disarm everyone with a sign. THEN act shocked when people don't see, care about, or choose to abide by that sign. THEN start here:

America 2016: If something bad happens, the government will lock you inside where the bad thing is happening, and not let you out where the bad thing is not happening. Because safety. Then when the bad thing is over, we perp walk everyone not accused of anything somewhere for processing.

My guess is fireworks and panic. With one witness being confused about what was going on. (Happens quite often) I can't imagine that such a place doesn't have decent security camera coverage, least enough to tell if there was a gun or not.

If people were fighting and they were shooting, one would assume it would at a person and not into a ceiling.

Hell I heard a 3rd hand story that they were shooting into the floor.

They secure the exits first. Then send people inside to deal with the threat.

You might not like it but in a place like that, it is a sensible strategy. There is a larger risk to society as a whole if the shooter were to escape and cause trouble elsewhere than it is to keep one contained and then dealt with accordingly.

How many do they have securing the exit? Do they have a swat team or Starfleet? Likely not. Likely it is billy badass with his badge and 'authoritative voice", who as far as I'm concerned can get out of my way or schedule the the surgery to have his badge removed from his rectum. A handful of folks who refuse to obey and there would be a beat down.
I don't care about the safety of the public and I don't particularly care what happens to Mr. L.E.O either, but me and my family's safety does and if Mr. L.E.O. Needs a lesson in this I would be happy to oblige.

And now all the bad guys know that the sheeple will lock themselves down in place for the slaughter if you make a loud noise in one of their precious holy places (Malls and schools, I guess).

When I was in high-school I was on the ground floor (actually a recessed level in the center of the ground floor) at 4 seasons mall with my girlfriend and my brother. Somehow a bunch of balloons popped on the floor above us while we were in a book store and everyone thought it was gunfire. I remember hitting the floor behind a shelf of books and then after a few seconds realizing what it was as we saw the balloon guy above us. Not saying it wasn't gunfire at Crabtree, but I can see how it could have been something else and folks thought it was gunfire.
